As this week's episode of "The Mob Doctor" begins, Grace is shooting pool in a bar with her boyfriend, Dr. Robinson. A couple of guys Grace knew in high school stop by the bar, and one of them falls off the bar and hits his head.
The next day, the guy with the head injury comes in. Grace staples his head wound shut, but then notices a yellow stain on his shirt. It's a "halo sign," a mass of cerebrospinal fluid. But tests show that the fluid isn't his: It belongs to one of his friends.
The friend, who is in the waiting room, gets very sick and is rushed into surgery to remove the blood from his brain. While the surgery is a success, Grace has to tell the man that he has a "time bomb" in his head, an area of his brain that has a 10 percent chance of rupturing and killing him. There is a risky surgery that could take care of the time bomb, but the man and his wife opt to take their chances.
Elsewhere, Constantine has tasked Grace with caring for a pregnant woman. The father is a politician, and Constantine plans to use his knowledge of the woman's pregnancy as a bargaining chip to ensure the politician supports him.
But after she gives birth to a healthy baby boy, the mother passes out and starts bleeding. Grace performs surgery on her, and the woman eventually wakes up. In order to save the woman's life, Grace had to do an emergency hysterectomy.
When the woman begs Grace to help her keep the baby, Grace gives her a plane ticket to get her out of town. Franco catches her in the act but agrees not to tell Constantine. Franco and Dr. Robinson finally cross paths, and Robinson is clearly not pleased to see Franco hanging out with Grace.
After Franco tells Grace to take her family and get out of Chicago, he goes to visit Moretti. Franco is then given an assignment: find a woman named Peggy, who will be Moretti's "ticket" back to Chicago. Franco finds her and follows her home: Peggy is then revealed to be Ralph Severino's daughter (Severino is the man who is set to testify against Moretti.)
Later on, Moretti puts a gun to Peggy's head and calls Severino over a video chat. Moretti says that Severino has a choice: kill himself or watch as Moretti kills Peggy. Severino kills himself, and Moretti says Peggy is dead if she squeals. With Severino dead, the feds have to drop their case against Moretti. Now, Moretti wants to take on Constantine and reclaim what belongs to him.
