Sam, Billie, Dan and Adam are friends and New Yorkers who struggle with eating disorders and tough it out in a rigorous support group called Belttighteners. While Dan considers getting his stomach stapled, Sam wants a date that's more like the svelte woman he saw in a commercial and Adam is on the prowl for unsuspecting food delivery people.
Sam begins to date his hydro-colonic therapist, but soon grows envious of her clientele. Billie's spends time with her parents, which stirs up an old resentment towards her father's ongoing pressure and high expectations. Meanwhile, Dan goes to see an S&M dominatrix when he starts to feel his marriage to Amy unraveling.
Sam injures himself while trying to groom his privates with scissors. Dan's wife kicks him out of the house when she discovers he's been trying some kinky sexual acts on the outside. Meanwhile, Billie becomes infatuated with the low body fat of a woman at the gym.
Sam goes out with a woman he has long been interested in, but the woman appears to be smitten with one of his friends instead. Meanwhile, when a bulimic man files a complaint against Adam, it throws the future of Adam's police career into question.
Sam tries to see the yoga instructor he met online by going to her class. Adam is busted by the Internal Affairs. Meanwhile, Billie begins to bond with Alison from the gym over Alison's cancer.
Sam's turn to veganism at the prodding of his new girlfriend causes him to reevaluate his dealings with all lifeforms. Meanwhile, Billie takes her new girlfriend home to meet the parents, while Dan realizes he has to get back into shape before he can have his gastric-bypass surgery.
Sam is coaxed into a new diet of fresh air and warm sunshine after his attempt at following Santi's vegan diet ends up adding pounds, and he begins to realize that he's in love with someone else. Meanwhile, Dan tries to keep from eating solid food by having his jaw wired shut, and Adam must admit to a binging and purging cycle that has not gone away.
